Hi there! Thank you for your interest in our 2024 Team Auditions. This is an opportunity to audition for our teams, or just a chance to take a class with us! To reduce paper and speed up our check-in process, we appreciate you filling out this registration form in its entirety prior to attending. Whether you are returning, or brand new to this space, we are excited to have you dance with us for a day, or maybe for the rest of the season! COMPANY WAIVER & RELEASE AGREEMENT:

- I hereby certify that I am age 18 years or older, and consent to my (or my child's) participation in re.DEFINE dance company events/activities. - I understand the physical activity that I/my child will endure as a participant of re.DEFINE Dance Company includes but is not limited to dance, auditions, rehearsals, classes, workshops, conventions, performances, and/or competitions. I am fully aware of the inherent risks involved with dance and group activities, and I volunteer myself/my child to participate in re.DEFINE dance company and affiliated events. - I assume full responsibility for any risks of loss, damage, injury, paralysis, or even death to myself/my child during the supervision of re.DEFINE. I further agree to hold harmless re.DEFINE Dance Company and affiliates, Soulidified Project and affiliates, as well as Impact Martial Arts directors and affiliates, for any/all loss, damage, injury that I/my child may incur during re.DEFINE events/activities. I know of no physical reason that would prevent myself/my child from participating in any re.DEFINE events/activities. - I understand that individuals should be covered by insurance policies held by myself/parents/guardians. - I understand that I/my child may be photographed or videotaped during re.DEFINE events/activities. I hereby release any footage of myself/my child to re.DEFINE dance company and its directors, for any/all promotional and informational uses. - I hereby release, waive, discharge, agree not to sue, and agree to hold harmless for any and all purposes re.DEFINE Dance Studio, Soulidified Project, Impact Martial Arts and affiliates, from any and all liabilities, claims, demands, or injury, including death, that may be sustained by myself/my child while participating in any re.DEFINE events or activities, or while on the premises of any other dance venue.
